SAP

Substance Abuse Professional Evaluations

Federally regulated evaluations for safety-sensitive employees covered by the U.S. Department of Transportation (DOT) following a positive test or other policy violation. Conducted in full compliance with 49 CFR Part 40.

· Initial face-to-face clinical evaluation
· Recommendation for education and/or treatment
· Follow-up evaluation upon completion
· Direct communication with your Designated Employer Representative

What to bring

Coming prepared makes the day easier.

Photo ID
A current government-issued ID.
Referral paperwork
Any documentation from your employer, DER, or attorney.
Relevant records
Prior treatment summaries, lab results, or court orders, if applicable.

Evaluations are scheduled privately and treated with the same respect as any clinical appointment. The goal is an honest, accurate picture — and a clear road back.
